Cancellation requests must be submitted
in writing and be received before May 15, 2011,
to be eligible for refund. A $100 service fee will be deducted
to cover administration costs. No refunds will be made for cancellations
received after May 15, 2011.
Attendees agree to hold harmless the Toronto Heart Summit
and the Facilities for any damages or injuries to property or
individuals that may occur during the conference.
